The Conway Area Lions Club Works to bring better vision and hearing to those in need, which is a major service goal for the Lions Club International, the NH Lions Sight and Hearing Foundation and our local Conway Area Lions Club. There are two different applications for assistance which are listed below, depending on your needs.

The Conway Area Lions Club has funding available to assist in the cost of an eye exam or eyeglasses for those in need. While the Conway Area Lions Club has partnered with Conway Eye Care to provide quality eye care and eye wear at no cost to the applicant, the Conway Area Lions Club will also provide funding towards eye exams and eyeglasses from alternate eye care providers. To be considered for assistance, please complete the application (2 pages) for Eye Care Assistance and submit to the address, fax or email listed above (also listed on the application form). If approved, payment will be made directly to your chosen eye care provider.
Should you need assistance with more costly items including hearing aids or medical eye care such as cataract surgery, the Conway Area Lions Club may sponsor your application to the NH Lions Sight and Hearing Foundation, which has greater funding available. This application is more detailed and requires supporting documentation to indicate your financial need, as well as an estimated invoice from an approved hearing/eye care provider indicating the cost of services needed. Within the application is the list of approved hearing and eye care providers, who should be contacted directly for consultation and estimated pricing. The application (13 pages) along with all supporting documentation and small application fee of $50 should be submitted to the above address. The Conway Area Lions Club will submit the application, along with a contribution from our local club, to the state NH Lions Sight and Hearing Foundation for consideration. If approved, your application fee and our club contribution go directly to the cost of the hearing aids/medical eye care. If denied for any reason, your application fee will be returned to you.
